Case Description:
A 66-year-old male was diagnosed with biopsy-confirmed squamous cell carcinoma of the left upper lobe. At initial presentation, the tumor measured 6.5 cm and was staged as T3N2. Endobronchial ultrasound confirmed involvement of mediastinal and hilar lymph nodes, specifically at levels 4L and 11L. Baseline pulmonary function tests showed an FEV₁ of 1.5 L (50% predicted) and a DLCO of 40%, making him a borderline surgical candidate. The patient underwent three cycles of chemoimmunotherapy, achieving an excellent radiologic response. A post-treatment PET/CT demonstrated no FDG-avid lymphadenopathy and a residual PET avid mass in the left upper lobe. Repeat VO₂ max testing following systemic therapy showed mild deterioration of pulmonary function test and VO₂ max testing was <10 ml/kg/min), making him ineligible for surgical resection. The patient is sent for consideration of radiotherapy.
